Interested in trying out for a sport?  Please go through Final Forms and create an account for your student.  You must do this in order to try out for any sport and upload the sports physical.  You can get these physical forms at any Dr office or Walk in clinic.  We do not have these forms at school.  Any questions please email, vguarnotta@mauryk12.org.
